Registration Fee
- One-Time Registration Fee: The one-time registration fee (multi-user) covers up to 6 household members. For each additional member beyond this limit, a charge of ₹99 per member applies. This fee is non-refundable and does not apply to subscription renewals.
- Renewal: No additional registration fee is required for the renewal of the subscription, provided the subscription is renewed before it expires.
Renewal and Cancellation
- Annual Renewal: Subscriptions are renewed automatically on an annual basis at the standard fee. Failure to renew results in automatic cancellation of the subscription at the end of the current term.
- Cancellation Policy: Subscriptions can be canceled at any time by notifying us in writing or through the designated platform. Cancellations are final and non-refundable. No pro-rata refunds will be issued for partial periods.
- Expired Subscriptions: Non-renewal will result in the expiration of the subscription. For expired subscriptions, renewal will require payment of both the standard subscription fee and the one-time registration fee, along with any additional charges for extra members. Expired subscriptions will be subject to the current terms and conditions at the time of renewal.
Service Compliance
- Legal and Ethical Standards: All services are provided in compliance with applicable legal and ethical healthcare standards. We are committed to maintaining the highest quality of care and adhering to professional guidelines.
- Service Providers: Services are delivered by qualified general practitioners at Qurocare Health Service (QHS). The credentials and qualifications of our service providers are verified and maintained to ensure competent care.
- Unlimited Healthcare Consultation: The unlimited healthcare consultations provided under the Qurocare Family Subscription are offered by General Practitioners or Physicians (Healthcare Specialists, HS) at Qurocare Health Service (QHS). These consultations do not include specialized care from Specialized medical professionals, such as cardiologists, orthopedists, or other specialists.
Service Exclusions and Additional Charges
- Consumables: The subscription covers consultations and basic services. Any and all consumables, including medications, diagnostic tests, medical equipment, and other specific items, will incur additional charges based on usage.
- Service Units: Unlimited healthcare under the subscription does not extend to other service units such as Qurocare Medical Specialty (QMS), Qurocare Diagnostic Services (QDS), Qurocare Pharmacy Services (QPS) or any specialized services outside of QHS.
- Additional Costs: Any additional services not explicitly included in the subscription, such as specialized treatments or procedures, will be charged separately.
Fee Adjustments
- Subscription Fees: The company reserves the right to change subscription fees. Subscribers will be notified of any changes with reasonable advance notice, typically at least 30 days before the new fee takes effect.
- Price Changes: Fee adjustments will be communicated through email, official notifications, or updates on the Qurocare platform.
Service Availability
- In-Clinic Traffic: Service quality and availability may be impacted by in-clinic traffic and the availability of staff. We aim to provide timely service but cannot guarantee uninterrupted availability due to these factors.
- Service Modifications: The scope of services and availability may change periodically. We will strive to inform subscribers of significant changes or updates to the services offered.
Offers and Discounts
- Terms of Offers: Any special offers or discounts are subject to specific terms and conditions, which will be clearly communicated at the time of the offer. These offers may be changed or discontinued at the company’s discretion without prior notice.
- Eligibility: Offers and discounts may have eligibility criteria that must be met. Subscribers should review the terms of each offer to understand how it applies to their subscription.
Service Rights and Conduct
- Refusal of Service: The company reserves the right to refuse service or terminate subscriptions for reasons including but not limited to violations of terms, misconduct, or failure to comply with policies. Subscribers who exhibit inappropriate behavior may be subject to immediate termination of their subscription.
- Dispute Resolution: Any disputes regarding the service or subscription will be addressed in accordance with our dispute resolution policies, which may include mediation or arbitration.
Professional Services
- Availability of Professionals: Services are provided by licensed and certified professionals. Availability of specific professionals may vary based on scheduling and operational needs. We will make reasonable efforts to accommodate requests for specific providers.
- Scope of Practice: The services provided are within the scope of practice of the healthcare professionals and are based on their qualifications and training.
Emergency Services
- Exclusions: Emergency medical services, including urgent care or critical care, are not covered under the subscription. Subscribers are advised to seek immediate attention from emergency services or hospitals for life-threatening or urgent medical conditions.
Liability Limitations
- Limitation of Liability: The clinic’s liability for any damages resulting from the subscription services is limited. The company is not liable for indirect, incidental, or consequential damages. Subscribers should review our liability policies for detailed information.
Health Information and Policies
- Accuracy of Information: Subscribers are required to provide accurate and complete health information to facilitate appropriate care. Failure to provide accurate information may affect the quality and effectiveness of the services received.
- Compliance: Subscribers must adhere to clinic policies, including appointment scheduling, cancellations, and other administrative procedures to ensure smooth service delivery.
Service Modifications
- Changes to Services: The company may modify, suspend, or discontinue services or terms at its discretion. Subscribers will be informed of significant changes in advance where possible.
- Notification: Subscribers will receive notifications of changes through email, official communications, or updates on the Qurocare platform.
Agreement
- Acceptance: By subscribing to Qurocare, you acknowledge that you have read, understood, and agree to these terms and conditions. Your continued use of our services constitutes acceptance of any modifications or updates to these terms.
Data Usage and Consent: By subscribing to Qurocare Family Membership, you consent to the collection, storage, and use of your medical data for the purpose of enhancing your healthcare experience and contributing to broader societal health benefits. This data may be used for research, quality improvement, and the development of new healthcare solutions, with strict adherence to privacy and confidentiality standards. Your data will be anonymized where applicable and will not be shared with third parties without your explicit consent, except as required by law.
Medical Consent Requirement
Signing the Medical Consent Agreement is mandatory for all members of the Qurocare Family Subscription. The subscription will be valid only with an active medical consent with Qurocare. By accepting these Terms and Conditions, the user also provides consent to the Medical Consent Agreement.
Applicability of Terms
These terms and conditions apply exclusively to the Qurocare Family subscription. Any other agreements or services outside of this subscription remain valid and unaffected.
